Pay: $56,000 to 65,000 per year contingent upon experience

The Community RN will be responsible for:
• Completing a monthly face-to-face assessment of the individuals with Developmental Disabilities and record any findings.
• Reviewing the individual consumer record monthly to include physician orders and the medication administration record.
• Maintaining communication with agency management
• Reporting and documenting of all changes in the individual’s health status and/or needs.
• Identifying needs for staff supervision /delegation or specialized instruction and any follow-up or plan of action that may result.
• Oversight of medication administration by certified medication aides, to include, but not limited to, the review of physician orders, medication administration record and staff documentation. This may also include periodic inspection of technique and skill of certified aides delegated to pass medications.
• Identifying nursing supports specific to individual consumer needs.
• Ensuring that specialized instruction/delegation regarding identified tasks is provided and documented.
• Oversight of drug destruction Records
• Be responsible for the accountability and documentation for activities dedicated to functions of Residential RN each month including:
• Completion of the Monthly Service Log which details time spent performing Residential RN functions each month
• Ensuring annual authorized units are delivered and documented
• Complete Abnormal Involuntary Movement Scale (AIMS) every 3 to 6 months (or more often if needed) for clients at risk of developing tardive dyskinesia (TD)
• Complete Health Risk Screening Tool (HRST) duties including:
• Completing HRST training through DMH
• Initial evaluation and completion of Health Risk Screening Tool for each residential client,
• Maintain Electronic Medical Record in DMH database system for all residential clients,
• Attend Individual Support Plan meetings for residential clients,
• Attend transition and discharge meetings from hospitals, rehabilitation facilities, nursing homes, etc.
• Provide a minimum of 24 hours per week of direct care to individual consumers on shift in the homes.
• Other duties as assigned
The Community RN will be responsible to:
• Account for their time dedicated to the functions of the Community RN Program each month. This will be accomplished by completion of the Monthly Service Log.
• Ensure the total hours available for each month are accounted for and do not carry over to the next month.
• Ensure the Monthly Service Log is completed accurately each month and submitted to their employer for review and maintenance of the document.
Qualities of a Community RN:
To effectively perform the duties of a Community RN, individuals must be able to demonstrate several competencies that are essential to the position, which include:
• Must be adept at problem-solving, including being able to identify issues and resolve in a timely manner
• Must possess strong interpersonal skills
• Must be able to communicate clearly, both written and orally, as to communicate with individuals with developmental disabilities and other co-workers.
• Must be able to effectively read and interpret information
• Must be able to prioritize and plan work activities as to use time efficiently
• Must be organized, accurate, thorough, and able to monitor work for quality
• Must be able to work in fast-paced environment
• Must be able to work well under stressful situations should that occur
• Must be dependable, able to follow instructions, respond to management direction, and must be able to improve performance through management feedback
• Computer skills
• Must be able to pass a background check that includes criminal history and abuse/ neglect
Job Type:
Work week will consist of Monday through Friday from 8:00 A.M. to 4:30 P.M. with a 30-minute lunch break On call for occasional emergencies by phone.
The following are paid holidays:
§ First day of January, New Year’s Day
§ Third Monday in February, Washington’s Birthday
§ Last Monday in May, Memorial Day
§ Fourth day of July, Independence Day
§ First Monday in September, Labor Day
§ Fourth Thursday in November, Thanksgiving Day
§ Twenty-fifth day of December, Christmas Day
When any of the specified holidays above fall on Sunday, those holidays will be observed on the following Monday. When any of the specified holidays above fall on a Saturday, those holidays will be observed on the preceding Friday.
Holidays are considered as a period of eight hours.
Qualifications
• Must hold a current Missouri State Board of Nursing RN license.
• Must be at least 18 years old
• Must hold a minimum of high school diploma or GED.
• Must complete Community RN training through the local DMH Regional Office
• Must be able to read and write in English in order to fulfill job requirements.
• Looking for an individual who has excellent people skills.
• Previous experience working with the elderly and/or disabled or must display a compassionate attitude for that constituent.
